Rituximab is a chimeric monoclonal antibody that targets the CD20 molecule expressed on the surface of B cells. It was first used in the treatment of non-Hodgkin’s lymphoma and later approved for the treatment of rheumatoid arthritis that does not respond adequately to disease-modifying antirheumatic drugs, including the anti tumor necrosis factor (TNF) inhibitors.
Rituximab depletes B cells by several mechanisms, including mediation of antibody-dependent cellular cytotoxicity, complement-dependent cytotoxicity, and B cell apoptosis. Rituximab is most efficacious and effective in patients who are seropositive and have failed one or more TNF inhibitors. It has a long-term safety of 9.5 years in RA.
